- Informed consent. You will receive clear information about your diagnosis, treatment, and prognosis.
- Refuse treatment. You have the right to refuse any or all treatment and to be informed of the potential consequences.
- Pain management. Your pain will be properly assessed and managed.
- Access medical records. Records may be requested and will be provided within a reasonable period.
- Privacy. Communications will be kept confidential.
Patients’ Responsibilities
Patients’ involvement is critical to the healing process. You are responsible for:
- Seeking medical attention promptly when needed.
- Providing accurate history including allergies, past illnesses, and medications.
- Complying with medical instructions, keeping appointments, and notifying staff of changes in condition.
- Accepting responsibility for consequences if you refuse treatment or fail to follow medical instructions.
- Arranging payment for all hospital and professional charges.
